Ten Health & Fitness has the best marketing of the five big UK physio and clinic brands compared, scoring 7.7 out of 10 on the Whito leaderboard.
Opinion and analysis. Every brand below was reviewed manually across seven marketing channels in August 2026, using publicly visible marketing activity. Scores are relative to the other brands in this comparison. Where a figure is not published, we say so.
The biggest physiotherapy chain in the UK barely posts on Instagram. It does not need to. Its growth comes from NHS contracts and insurer referrals, not from anyone scrolling on a sofa.
Meanwhile the best marketer in this comparison barely does physio. It is a fitness lifestyle brand with a clinical team inside it, and it markets like one. Full pricing, paid social funnels, retention memberships.
That is the real lesson of this leaderboard. Marketing channels follow the business model, not the other way round. A clinic funded by insurer contracts markets differently to a clinic that has to win every patient one card payment at a time.
We scored five of the biggest names in UK private physiotherapy to show exactly how that plays out.
The leaderboard
| Rank | Brand | Social | Website | SEO | Paid | Reviews | Brand | Average | |
|---|---|---|---|---|---|---|---|---|---|
| 1 | Ten Health & Fitness | 9 | 9 | 8 | 6 | 7 | 7 | 8 | 7.7 |
| 2 | Pure Sports Medicine | 8 | 8 | 7 | 8 | 5 | 6 | 8 | 7.1 |
| 3 | Ascenti | 5 | 8 | 4 | 9 | 5 | 9 | 9 | 7.0 |
| 4 | Bodyset | 6 | 9 | 7 | 6 | 6 | 7 | 7 | 6.9 |
| 5 | Six Physio | 7 | 7 | 4 | 8 | 4 | 9 | 8 | 6.7 |
A note on the Paid column. No physio brand publishes its ad spend, so these scores are inferred from visible signals only, such as ad library activity and landing pages built for campaigns.
Company verdicts
Ten Health & Fitness (7.7): the consumer marketer
Ten is the only brand in this set publishing full pricing, checked August 2026. Classes from £24, memberships from £110 to £345 a month, and an intro offer of 3 classes for £57, which is a classic paid social funnel. Its Circle of Care model links physio, clinical exercise and Pilates into one retention journey, so a patient becomes a member rather than a one off booking. It backs this with branded apps, a newsletter, and active Instagram, TikTok and YouTube accounts, though follower counts are not published. The weakness is SEO. There is little condition led content, so Ten rents attention through paid and social rather than earning search demand.
Pure Sports Medicine (7.1): the authority player
Pure runs ten clinics with 120 plus clinicians and direct billing for Bupa, AXA Health, Vitality, WPA, Aviva and Healix. Its advice hubs on running, cycling, ACL injuries and women’s health are proper SEO assets, not blog filler. The free 15 minute consultation and live chat make for unusually good conversion design for a clinic. The review picture is split though. Its Kensington clinic sits around 4.96 on Doctify from about 100 reviews, but the brand sits around 3.7 on Trustpilot from roughly 430. Self pay pricing is not published.
Ascenti (7.0): the distribution giant
Ascenti is the UK’s largest private employer of physiotherapists, with 300 plus sites. Its growth engine is NHS contracts and insurer pipelines, and in March 2026 it piloted a new digital physio app with the York and Scarborough NHS trusts. Trustpilot sits around 4.7 from roughly 6,300 reviews, comfortably the biggest managed review base in this set. It also has the strongest organic footprint, with a large advice library and hundreds of location pages. Consumer social and email are the gaps, with no visible newsletter at all.
Bodyset (6.9): the transparency play
Bodyset rebranded from Capital Physio in February 2020 and now runs 40 plus clinics, claiming 500,000 plus treatments delivered. It is the only physio brand publishing full regional self pay pricing, from a £93 initial assessment in Greater London down to £65 in the Midlands and North, with 6 for 5 bundles on top. That transparency does real commercial work for a self pay patient comparing tabs. The problem is the review split. Bodyset claims 4.9 on Google from over 2,000 reviews, but Trustpilot sits around 3 on low volume, a neglected channel quietly undercutting the trust story.
Six Physio (6.7): the heritage brand
Founded in 1996, Six Physio runs 14 locations with 100 plus physios. Ascenti acquired it in December 2022, but it still trades as its own brand. Its homepage badge shows 4.9 from over 10,000 aggregated ratings, the biggest social proof number in the set. A 30 year old domain and a decent knowledge hub give it solid London SEO. But there is no published pricing, no visible newsletter, and no obvious paid activity, checked August 2026.
What the channels show
Reviews are split across platforms, and most brands only manage one
Ascenti owns Trustpilot. Bodyset claims Google. Pure wins Doctify but sits at 3.7 on Trustpilot. Every brand here has one strong platform and at least one weak or ignored one. An unmanaged review platform quietly taxes every other channel, because patients check it right before they book.
Price transparency is rare, and it is a differentiator
Only Bodyset and Ten publish what treatment costs. Everyone else makes you enquire. For a self pay patient comparing three tabs, the published price usually wins the click, because it removes the one question that stops a booking.
The biggest player has the smallest consumer voice
Ascenti grows through NHS and insurer pipes, so its consumer marketing is thin by design. The consumer brands invert this, all voice and no contracts. Nobody in this set does both, which tells you how much channels follow the business model.
Email is the weakest channel across the board
The average patient needs 4 to 6 sessions, then lapses. Two of five brands have no visible newsletter at all. Retention email is revenue left on the table, because a former patient with a new niggle is the cheapest booking a clinic will ever get.
Key lessons for any local clinic
Publish your prices
Only two of five national chains do it, so a local clinic that publishes prices immediately looks more trustworthy than most of the market. Hidden pricing loses self pay bookings you never see.
Pick one review platform and flood it
Own your local Google profile and ask for a review at discharge, every time. One platform with hundreds of recent reviews beats three platforms with a handful each.
Build condition pages, not just a services list
People search for their problem, not your service menu. A page for knee pain, one for lower back pain, one for tennis elbow. That is how Ascenti and Pure earn search traffic the others pay for.
Email your former patients
A quarterly check in note with a rebooking link is enough. Your past patient list is the highest converting audience you own, and most clinics never contact it again.
Make booking frictionless before spending on ads
Online booking, visible prices, fast mobile pages. Paid traffic sent to a clunky enquiry form is money burned. Fix the funnel first, then buy attention.
Steal this playbook
Five moves any clinic can make this month, in order.
- Put your initial assessment price on your homepage and booking page this week.
- Claim your Google Business Profile, then ask every discharged patient for a review by text within 24 hours.
- Write one condition page for the problem you treat most, with symptoms, what treatment involves, and a booking link.
- Export your past patient list and send one plain email offering a check in appointment.
- Test your own booking journey on a phone. Count the taps from homepage to confirmed appointment, then remove at least two.
Common questions
Who is the biggest physiotherapy provider in the UK?
Ascenti, the largest private employer of physiotherapists in the UK, with 300 plus sites. It holds roughly 6,300 Trustpilot reviews at around 4.7, figures approximate as of August 2026.
How much does private physiotherapy cost in the UK in 2026?
Published chain prices run from about £65 for an initial assessment in the Midlands and North to £93 in Greater London, based on Bodyset’s published rates. Many clinics do not publish prices at all, which makes comparison harder than it should be.
Do physio clinics win business from social media?
The picture is mixed. The strongest social brand in this set is really a fitness lifestyle brand with physio inside it. Clinical providers mostly grow through search, GP referrals, NHS contracts and insurer routes, where social plays a supporting role at best.
What matters more, Google reviews or Trustpilot?
Google, for local intent, because it is what a patient sees when they search for a clinic near them. Two national chains in this comparison score below 4 on Trustpilot while claiming 4.9 elsewhere. Manage the platform patients actually see.
Are independent clinics being pushed out by chains?
Consolidation is real. Ascenti bought Six Physio in 2022, and Spire bought Vita Health Group for around £74m in October 2023. But chains compete on contracts and coverage, not local relationships, and that leaves plenty of room for a well run independent clinic.
