Someone wakes up with a back that will not straighten. The NHS route means a GP appointment, a referral and a wait measured in weeks. So they do what a fast growing number of people in pain now do: they ask ChatGPT or Google’s AI answer for a good private physio nearby, and get two or three clinic names back.
If your clinic is not one of them, the booking goes elsewhere before you ever knew it existed.
Independent physios face the same headwind here as independent vets and dentists. The national players, the likes of Ascenti, Pure Physiotherapy and Bodyset, run hundreds of consistent, complete listings. Our physio clinics leaderboard scored the biggest names in the sector, and the pattern was clear: the winners are not the best clinicians, they are the most legible businesses. AI rewards legibility.
The good news is that legibility is not a budget question. A single clinic can be as clear as a 300 clinic group.
What people in pain actually ask AI
Physio near me for lower back pain. Sports physio who treats runners. Private physio cost without a GP referral. Physio for a frozen shoulder in the town name. Can I see a physio without a referral. Home visit physio for my elderly mum.
Two things stand out. First, people ask by condition, not by clinic name. Second, they ask practical questions about referrals, price and availability. If your website answers those questions in plain text, you are the answer AI reaches for. If it says holistic care tailored to you, you are not.
Why AI leaves clinics out
Your Google Business Profile
The first source AI tools check for local healthcare. It needs the right category, real photos of the clinic and treatment rooms, accurate hours, and services listed by name: sports injury, post surgery rehab, MSK physiotherapy, acupuncture, home visits. A profile that just says physiotherapist gives AI nothing to match against a question about a frozen shoulder.
Your reviews
Health decisions push AI tools further towards trusted, reviewed providers. Volume and recency both matter. The clinics that win ask for a review at discharge, when the outcome is fresh and the gratitude is real. A simple habit, ask every discharged patient the same day, will outperform any amount of advertising within a year.
Your registration
Physiotherapist is a protected title in the UK, and every practising physio must be registered with the HCPC. Most are also chartered members of the CSP. Patients rarely check. AI tools do, in the sense that they favour providers whose credentials are stated and verifiable. Put registration numbers on your about page in text. It costs nothing and it separates you from the unregulated massage and therapy noise around you.
Clear conditions and prices
Most clinic websites list treatments. Almost none list conditions, and fewer still list prices. But the question is always about a condition, and self paying patients always want the price. A page per condition you treat, in plain English, with your initial assessment fee stated, is the single most quotable asset a clinic can build.
What to fix, in order
- Complete your Google Business Profile with services by name, photos, hours and your booking link.
- State plainly on your website that patients can self refer, what the first appointment costs and how soon you can usually see someone. Speed is a selling point against NHS waits.
- Add your HCPC registration and CSP membership in text, with names, on your about page.
- Ask every discharged patient for a Google review the same day, and reply to all of them.
- Build one plain page per major condition: back pain, sports injuries, post surgery rehab, whatever you actually treat most.
Common questions
Why does AI keep recommending the big physio chains?
Because their information is complete and consistent everywhere. Hundreds of identical, detailed listings look reliable to a machine. An independent clinic can match that standard for its own area in an afternoon, and a named clinician with strong local reviews often beats a faceless national brand once the basics are level.
Do I need to publish prices to get recommended?
You will not be excluded for hiding prices, but you will lose the self pay patients who ask cost questions directly. AI tools quote providers who state fees plainly. Publishing your initial assessment price also filters out the phone calls that were never going to book.
